Billing/Payments
- All payments are due at the time of the visit.
- Membership fees are billed on the first day of each month.
- Payments accepted via Health Savings Account (HSA) or credit/debit card.
- You will receive an itemized receipt for the services provided by Bloom Pediatrics. This receipt can be submitted to your insurance provider for reimbursement as an out-of-network claim. If you would like assistance with your claim we recommend using www.reimbursify.com.
Travel Fees
- No associated travel fees within St. John’s or Duval Counties.
- Nassau, Clay, or Putnam counties: Additional $25 fee
